Human parainfluenza virus type 1 (HPIV-1) is a significant respiratory pathogen, particularly affecting children, immunocompromised individuals, and the elderly. The virus is primarily transmitted through direct contact with infected respiratory secretions or via aerosolized droplets. Upon entry, HPIV-1 targets epithelial cells lining the upper and lower respiratory tracts and binds to cell surface sialic acid receptors via its hemagglutinin-neuraminidase (HN) glycoprotein. The virus replicates in the cytoplasm, causing cell lysis and the release of new virions, which then spread to adjacent tissues. The immune response to infection typically leads to inflammation, causing symptoms such as fever, cough, and runny nose, and in severe cases, croup (laryngotracheobronchitis). HPIV-1 is a leading cause of croup in young children, characterized by a distinctive barking cough and wheezing. While most infections are self-limited, severe cases may require hospitalization, particularly in infants or immunocompromised patients.
HPIV-1 is an enveloped, single-stranded, negative-sense RNA virus belonging to the Paramyxoviridae family and the Respirovirus genus. Viral particles are pleomorphic, typically 150-300 nanometers in diameter, and enclosed in a lipid bilayer derived from the host cell membrane. The envelope is studded with two key glycoproteins: hemagglutinin-neuraminidase (HN), which mediates viral attachment and entry into host cells, and fusion protein (F), which facilitates fusion of the viral and host cell membranes. Beneath the envelope lies the matrix protein (M), which maintains the structural integrity of the virus and connects the nucleocapsid to the envelope. The viral genome consists of approximately 15,000 nucleotides and encodes six major structural proteins: nucleoprotein (N), phosphoprotein (P), matrix protein (M), fusion protein (F), hemagglutinin-neuraminidase (HN), and large polymerase protein (L). The RNA genome is tightly bound to the N protein to form the helical nucleocapsid and serves as a template for replication and transcription by the viral RNA-dependent RNA polymerase (composed of the L and P proteins).
