Abnormalities in paravertebral muscle (PVM) play a significant role in the pathogenesis of idiopathic scoliosis (IS); although elevated oxidative stress may contribute to PVM damage in IS patients, the specific mechanisms underlying this oxidative stress remain unclear. Here, researchers found that primary skeletal muscle mesenchymal progenitor cells (hSM-MPCs)-key cells in vertebrate skeletal muscle myogenesis-derived from IS patients exhibited increased apoptosis, impaired myogenesis, and elevated oxidative stress. RNA sequencing and subsequent analysis revealed a significant upregulation of myostatin (MSTN) expression in hSM-MPCs from IS patients. Overexpression of MSTN in control hSM-MPCs led to increased NADPH oxidase 4 (NOX4) expression, promoted reactive oxygen species (ROS) production and apoptosis, and inhibited myogenesis; conversely, knocking down MSTN in hSM-MPCs from IS patients reduced NOX4 expression, suppressed ROS production and apoptosis, and promoted myogenesis. Furthermore, MSTN overexpression in mouse PVM induced elevated oxidative stress and scoliosis without accompanying vertebral structural abnormalities. Collectively, these findings suggest that the abnormal changes in PVM and the accumulation of oxidative stress observed in IS patients may stem from MSTN upregulation, a process that likely contributes to the onset and progression of IS.

To verify the effects of MSTN upregulation, researchers constructed MSTN-overexpressing CT-hSM-MPCs (Figure 1A and 1B). As shown in Figure 1C, MSTN overexpression led to increased Bax protein levels and the Bax/Bcl2 ratio, but did not alter Bcl2 levels. Compared to control cells, the proportion of TUNEL-positive cells was significantly increased in MSTN-overexpressing CT-hSM-MPCs, while the proportion of EdU-positive cells was decreased (Figure 1D and 1E). Furthermore, MSTN-overexpressing CT-hSM-MPCs showed increased ROS production (Figure 1F). The results also showed that MYOG mRNA and protein levels were decreased in MSTN-overexpressing CT-hSM-MPCs, while NOX4 expression was significantly increased (Figure 1G and 1H). However, NOX2 protein levels were downregulated in MSTN-overexpressing CT-hSM-MPCs (Figure 1H).
