Wild-type human adenovirus serotype 21 (HAdV-21) belongs to the Adenoviridae family and is primarily transmitted through respiratory droplets, direct contact with infected individuals, or contact with contaminated surfaces. Fecal-oral transmission can also occur, particularly in densely populated or unsanitary environments. HAdV-21 is known for its respiratory tropism, often causing mild to moderate upper respiratory tract infections, including pharyngitis, bronchitis, and conjunctivitis. In immunocompromised individuals or young children, the virus can cause severe complications such as pneumonia and even systemic infection. The virus enters host cells through endocytosis mediated by the coxsackievirus and adenovirus receptor (CAR) and then replicates within the cell nucleus. Its pathogenesis involves lytic infection, triggering inflammation and cell death, leading to clinical symptoms.
HAdV-21 is a non-enveloped, icosahedral virus approximately 70-90 nanometers in diameter. Its capsid is composed of 240 hexon trimers and 12 penton bases at the apex. Each base is modified with a fiber protein to facilitate host cell attachment. The fiber protein binds to the CAR, while the penton bases interact with integrins to mediate cellular internalization. The viral genome is a linear double-stranded DNA molecule of approximately 36 kilobases, encoding approximately 30-40 genes. These genes are organized into early (E1-E4), middle, and late (L1-L5) transcription units that regulate viral replication, host immune evasion, and structural protein synthesis. Early genes regulate cell cycle progression and antagonize host defenses, while late genes produce capsid components. The genome also contains inverted terminal repeats (ITRs) essential for replication and a virus-associated RNA (VA RNA) that counteracts the host interferon response. The genetic stability and efficient replication machinery of HAdV-21 make it a potent pathogen and a potential gene therapy vector.
